Sports Physicals

Sports Physicals are provided to the student athletes at no charge on scheduled dates throughout the school year. The website will be updated with the sports physical dates, once they are scheduled.  Students must have a current sports physical to participate in all Windsor sports. Please check with the school nurse for your students sports physical status. 
 

FOR STUDENTS WHO NEED A SPORTS PHYSICAL THAT ELECT  NOT TO HAVE THEM DONE AT SCHOOL (OR MISSED THE PHYSICALS):

THE STUDENT pre-screen examination form and health-exam-form WILL NEED TO BE TAKEN AND FILLED OUT BY THE DOCTOR AND TURNED INTO THE NURSE. 
TWO OPTIONS: PARENTS ARE RESPONSIBLE FOR COST IN BOTH

 1) PRIVATE PHYSICIAN: PARENTS MAY TAKE THEIR CHILD TO THE PHYSICIAN OF THEIR CHOICE- DOCUMENT IS ATTACHED THAT NEEDS TO BE COMPLETED BY THE PRIVATE PHYSICIAN-ONCE COMPLETED IT GETS RETURNED TO THE NURSE MS/HS  TO SIGN OFF ON FAMILY ID

 2) WINDSOR SCHOOL PHYSICIAN: LOURDES OCCUPATIONAL HEALTH OFFICE
