The Windsor Central School District will host a Walgreens Covid-19/Flu Vaccination Clinic on October 7^th. Community members, WCSD employees and students 12 and over are eligible. All minors wishing to be vaccinated must be accompanied by parent or guardian.            Location 1:  Windsor Central Middle School     TIME:  11:00am - 1:00pm
           Location 2:  CR Weeks Elementary School        TIME:  2:00pm - 5:00pm
 
 - WHO:      
           Students 12 years of age and older (must be accompanied by parent/guardian)
           WCSD Employees
           Community members
 
 - WHAT TO BRING:
            Insurance Information
            Proof of ID
            Completed "Walgreens Vaccine Administration Record" (blank forms available at clinic site)
 
 ALL MINORS WISHING TO BE VACCINATED MUST BE ACCOMPANIED BY THEIR PARENT/GUARDIAN
